Otolaryngology ARCP

Otolaryngology ARPCSs will be held on Thursday 26th September 2019 at the Mercure Grand Hotel, Broad Street, Bristol BS1 2EL
Please sign up. After your surname please add - AM or PM so we know whether you wish to attend in the morning or afternoon session. A definite time will be emailed to you in due course. Please arrange the necessary leave of absence from the workplace.
All paperwork needs to be uploaded to your ISCP account, at least 4 weeks before the event.
A copy of the Form R needs to be filled in and sent in to SEVSurgery.School.sw@hee.nhs.uk at least 2 weeks before the event.
Please ensure the following is completed:

ePortfolio - you must ensure that your ePortfolio is up to date by the date specified above. Please ensure that you have checked JCST website for certification guidelines and waypoint and penultimate checklists with regards to the relevant targets for your year of training.

Educational supervisors report – up to date.

Form R for revalidation purposes –to be signed and all parts completed. Don’t forget to add

Wider Scope of Practice Form – if applicable.
All the above documents must be completed and uploaded on ISCP/portfolio Please ensure that you have checked your ARCP decision aid with regards to the relevant targets for your year of training. You can now upload Form R’s to ISCP as follows:
Add > OTHER EVIDENCE > Miscellaneous. If you need more space, then you will need to request this by emailing the ISCP helpdesk directly.
